Incident Report
Use this form to record significant injuries, safety/behavior issues, or property damage. While we ask for pool staff name, please note the names will be used by the board for reference and follow-up questions, they will be kept as confidential as possible.
For Behavioral or Safety issues, each employee who was a witness should complete the form.
